[Enable Fail-Open State for DUO MFA when DUO is unavailable]

If DUO is unavailable, either due to a DUO Outage, or local internet outage, make DUO fail-back to open.

(ie ping duo.com, if unavailable bypass duo or fail over to another MFA option)

 

Saves giving out emergency access to non-admins with PasswordState, or getting calls at random hours.
